Traveling Consultant Pharmacist — Fort Gratiot, MI | Full-Time | Salaried | Travel Required

$125,000-$132,000 annually

We're seeking a compassionate and clinically strong Traveling Consultant Pharmacist to support our team and the PACE program in Fort Gratiot, MI. In this role, you'll be a key member of the interdisciplinary care team, ensuring safe, effective, and appropriate medication use for elderly participants with complex medical needs.

• Review and manage participant medication regimens
• Participate in interdisciplinary team (IDT) meetings and care planning
• Conduct medication reconciliations and chart reviews
• Identify and resolve medication-related issues
• Ensure compliance with CMS, state pharmacy laws, and PACE guidelines
• Educate participants, caregivers, and care team members

  D from an accredited school
• Active, unrestricted pharmacist license
• 2+ years of experience in geriatrics, LTC, or similar setting
• PACE experience and BCGP or Consultant Pharmacist certification preferred
• Willingness to travel
