ABOUT THE ROLE:The RN Case Manager is responsible for the necessary functions of ensuring a successful move-in of all future residents.
Here are a few of the daily responsibilities of a RN Case Manager:- Performs resident evaluations as required by state regulations and/or Community policy.
- Assists and guides residents and families with the move-in process and orients them to the community, including reviewing the resident handbook.
- Assists residents and families with accessing services within the Community and outside the Community.
- Discharge planning and assistance with the move-out process.
- Participates in the development and implementation of each resident's individual service plan.
- Documents case management notes in the resident file as necessary.
- Communicates with members of the management team to ensure coordination of services to the residents.
- Oversees and coordinates the Individualized Service Plan (ISP) and the ALR personal data and resident evaluation form.
- Performs resident evaluations as required by state regulations and/or Community policy.
- Supports Community guidelines and state regulations for admission and retention of residents.
- RN currently licensed by the State of Florida in good standing.
- At least 6 months nursing experience. Geriatric involvement preferable.
- Can generate a warm, friendly, and caring manner on first impression.
- Can communicate effectively with residents, families, staff, and visitors.
- Maintain the highest level of confidentiality.
- Can pass a pre-employment drug screen and criminal background check.

Full-Time, Monday-Friday 8:00AM-4:30PM
Salary: from $32 per hour
