Job Opportunity:
Lead MDS Coordinator, RN
Facility: Charlotte Bay Rehab and Care Center
Location: Port Charlotte, Florida
Position Type: Full-Time, 8:00 AM – 4:00 PM
Salary: $42–$46 per hour, depending on experience
Responsibilities:- Quality Care Oversight: Ensure the delivery of high‑quality care to our long‑term care residents.
- Comprehensive Assessment: Conduct thorough initial interviews and assessments for new residents.
- Care Planning: Develop individualized care plans focused on both short‑term and long‑term goals.
- Interdisciplinary
Collaboration:
Lead meetings with multidisciplinary teams, caregivers, and families to coordinate resident care. - Community Engagement: Work with specialists to optimize resident outcomes.
- Leadership &
Education:
Guide nursing staff, communicate effectively at all levels, and conduct in‑service education on MDS, RAPS, and Triggers. - Schedule Adherence: Maintain compliance with the MDS/Care Planning schedule for non‑Medicare residents.
- Florida RN License: Valid and in good standing.
- Skilled Nursing
Experience:
Minimum of 2 years in a Skilled Nursing Facility. - Expertise: Strong knowledge of MDS, Medicare, and the care‑planning process.
