The Patient Aligned Care Team (PACT) Outpatient Registered Nurse (RN) is responsible for delivering evidence-based, gender-specific care to primary care patients and supervising licensed vocational nurses and nursing assistants. This role involves providing direct patient care and care coordination within the Primary Care Department for Veterans and their families. The nurse manages assigned patient panels, utilizes registries, and ensures timely assessment and access for patients.
Responsibilities include health promotion, health education on wellness, disease prevention, and chronic disease management. The nurse will also assist in coordinating nursing education, orientation, competency development, and participate in quality improvement initiatives and outcomes analysis
Duties include but are not limited to:
* Enhance Veteran and patient-centered care by addressing experience-related challenges, including care coordination, timeliness, process improvements, common areas of dissatisfaction, and environmental concerns within the immediate practice setting.
* Serves as an advocate for patients, families, and caregivers to enhance outcomes within the immediate practice setting.
* Lead the implementation of high-reliability principles and I CARE values to enhance the consistency of care provided and improve patient outcomes.
* Lead patient care improvement through monitoring, analyzing, and evaluating care outcomes in the immediate practice setting.
* Lead teams, participates in team meetings, and actively collaborates throughout all stages of patient care planning. Effectively integrates goals, opportunities, and resources.
* Implement evidence-based practices and research to enhance outcomes within the immediate clinical setting.
Preferred Experience:
Two years of experience as an RN in a collaborative team environment, including clinic, urgent care, and outpatient care settings.

Pay:
