Job Summary
The Primary Care Mental Health Integration (PCMHI) Registered Nurse (RN) is responsible for coordinated patient care, patient education, and ensuring customer satisfaction throughout the continuum of care. This role adheres to established policies and promotes positive care outcomes through collaboration with interdisciplinary team members. Key responsibilities include contributing to quality improvement initiatives and supporting customer service initiatives.
Responsibilities- The PCMHI Collaborative Care Manager (CoCM) Registered Nurse delivers competent, evidence-based care to assigned patients in the outpatient primary care setting.
- Supports coordination of nursing education, orientation, competency development, and provides consultation on quality improvement and outcomes utilization.
- Delivers evidence-based, condition-specific (e.g., depression), protocol-driven services to collaboratively assist PACT providers in managing mental health and health behavior concerns.
- Provides patient education, activation, behavioral interventions, and monitors medication adherence and responses.
- Engages in patient outreach and engagement to ensure appropriate treatment; reviews, develops, maintains, and monitors Veteran-specific care plans with the interdisciplinary team to ensure accurate reporting and validation of patient outcomes.
- Participates in caseload consultations and delivers education and brief interventions using evidence-based techniques such as behavioral activation and motivational interviewing.
- Includes relapse prevention planning throughout the care process, including transitions from active treatment to maintenance as goals are achieved.
- Communicates effectively with patients, families, caregivers, VA leadership, and interdisciplinary team members using therapeutic communication and de-escalation techniques.
- Collaborates with PCMHI leadership and team members to develop and enhance educational materials and marketing strategies to promote PCMHI CoCM services.
- Works with the team to ensure the delivery of high-quality, Veteran-centered services at both the patient and program levels.
- Minimum of two years of experience as a licensed registered nurse, including at least one year in an outpatient clinic setting with a multidisciplinary mental health team.
- Demonstrated experience collaborating across multiple departments; ability to lead group discussions and deliver presentations in-person and in virtual formats.
