Job Summary
The Licensed Social Worker (LMSW) provides professional social work services to patients and families at Poplar Bluff Regional Medical Center. You will address psychosocial needs, support care transitions, and connect individuals to vital community resources. This role performs comprehensive assessments and collaborates with an interdisciplinary team to implement effective discharge plans, improve patient outcomes, and support vulnerable populations.
Role FocusYou will manage Social Work discharge planning needs including:
- Placements & Guardian ships
- Social Determinants of Health & Homelessness
- Maternal-Child (Mom/Baby) & Geriatric Populations
- Conduct age-appropriate psychosocial assessments for neonatal, pediatric, adolescent, adult, and geriatric populations.
- Identify barriers to recovery and discharge by assessing the relationship between psychosocial factors and medical conditions.
- Provide emotional support, crisis intervention, and counseling for illness, injury, grief, or disability.
- Develop and document individualized discharge plans in coordination with the interdisciplinary team.
- Manage complex, high-risk referrals involving suspected abuse, adoption, fetal demise, end-of-life care, and behavioral health.
- Coordinate transitions of care, including home health, hospice, rehabilitation, and long‑term care placement.
- Maintain timely, accurate documentation per regulatory and organizational standards.
- Participate in interdisciplinary rounds with physicians, nurses, and case managers.
- Education
:
Master’s Degree in Social Work (MSW) or Psychology required. - Experience
: 2–4 years of experience in a healthcare or clinical social work setting preferred. - Licensure
: LMSW (Licensed Master Social Worker) – Missouri State Licensure required. - Certification
: BCLS (Basic Life Support) must be obtained within 7 days of employment.
- Expertise in psychosocial assessment and healthcare discharge planning.
- Strong understanding of community resources and social service systems.
- Proven counseling, conflict resolution, and communication skills.
- Ability to manage emotionally complex situations with empathy and professionalism.
- Proficiency with electronic medical records (EMR) and case documentation.
