Intake Coordinator (LPN) (Healthcare)We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to serve as an Intake Coordinator for our Home Health and Hospice programs. This role is critical in ensuring a smooth and timely intake process for patients, families, referral sources, and clinical staff. The Intake Coordinator is responsible for managing referrals, verifying eligibility, coordinating start-of-care services, and supporting regulatory and documentation compliance.
Responsibilities- Receive, review, and process referrals for home health and hospice services
- Verify patient eligibility, insurance coverage, and authorization requirements
- Communicate with referral sources, patients, families, physicians, and internal clinical teams
- Collect, review, and enter accurate patient information into the EMR
- Coordinate start-of-care visits and clinician assignments
- Ensure compliance with Medicare, Medicaid, and payer guidelines
- Maintain clear, timely documentation in accordance with agency policies
- Serve as a clinical resource during the intake process
- Provide excellent customer service and act as a liaison between departments
- Current Licensed Practical Nurse (LPN) license in good standing
- Minimum 1 year of experience in home health, hospice, or related healthcare setting preferred
- Strong knowledge of intake, referral, and admission processes
- Familiarity with Medicare, hospice eligibility criteria, and home health regulations
- Proficiency with EMR systems and basic computer applications
- Excellent communication, organizational, and multitasking skills
- Ability to work independently and collaboratively in a fast-paced environment
- Prior intake or admissions experience in home health or hospice
- Knowledge of insurance verification and authorization processes
- Experience coordinating care with interdisciplinary teams
