Intake Coordinator (LPN) (Healthcare)

We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to serve as an Intake Coordinator for our Home Health and Hospice programs. This role is critical in ensuring a smooth and timely intake process for patients, families, referral sources, and clinical staff. The Intake Coordinator is responsible for managing referrals, verifying eligibility, coordinating start-of-care services, and supporting regulatory and documentation compliance.

Responsibilities
  • Receive, review, and process referrals for home health and hospice services
  • Verify patient eligibility, insurance coverage, and authorization requirements
  • Communicate with referral sources, patients, families, physicians, and internal clinical teams
  • Collect, review, and enter accurate patient information into the EMR
  • Coordinate start-of-care visits and clinician assignments
  • Ensure compliance with Medicare, Medicaid, and payer guidelines
  • Maintain clear, timely documentation in accordance with agency policies
  • Serve as a clinical resource during the intake process
  • Provide excellent customer service and act as a liaison between departments
Qualifications
  • Current Licensed Practical Nurse (LPN) license in good standing
  • Minimum 1 year of experience in home health, hospice, or related healthcare setting preferred
  • Strong knowledge of intake, referral, and admission processes
  • Familiarity with Medicare, hospice eligibility criteria, and home health regulations
  • Proficiency with EMR systems and basic computer applications
  • Excellent communication, organizational, and multitasking skills
  • Ability to work independently and collaboratively in a fast-paced environment
Preferred Experience
  • Prior intake or admissions experience in home health or hospice
  • Knowledge of insurance verification and authorization processes
  • Experience coordinating care with interdisciplinary teams
