An exciting opportunity has arisen for an Advanced Nurse Practitioner to join our friendly, supportive and forward thinking Primary Care Network (Poole North) in a part time role covering two practices Canford Heath Group Practice and Birchwood Medical Centre.
The ideal candidate will be able to work a Wednesday morning and all day Friday due to rooms and will be a prescribing ANP.
Although working across two practices, we have a team of ANPs within the network who support each other through regular communication and quarterly lunch meetings.
As well as Poole North Primary Care Network being a great place to work, both Canford Heath Group Practice and Birchwood are friendly and supportive and regular supervision will be put in place, both for on the day need and ongoing development.
The ideal candidate will have obtained their Digital Badge or be willing to work towards it.
Main duties of the jobAn Advanced Nurse Practitioner (ANP) in a GP practice works autonomously to assess, diagnose, treat, and manage patients with a wide range of acute and chronic health conditions. They play a key role in delivering primary care services and supporting the wider multidisciplinary team and they;
- Conduct face-to-face, consultations for acute patients.
- Perform physical examinations.
- Assess undifferentiated and complex presentations.
- Order and interpret diagnostic tests (e.g., blood tests, ECGs, X-rays).
- Make clinical decisions independently or collaboratively with GPs when required.
- Prescribe medications as an independent prescriber.
- Initiate, adjust, or discontinue treatments.
- Provide advice on medication management and side effects.
- Assess and treat minor injuries and acute conditions such as infections, respiratory illnesses, and exacerbations of chronic disease.
- Identify red flags and elevate care when necessary.
- Conduct annual reviews and create personalised care plans.
- Provide lifestyle advice (smoking cessation, weight management, exercise).
- Promote screening programmes and preventative health measures.
- Refer patients to secondary care or specialist services when needed.
- Liaise with community services, district nurses, and allied health professionals.
- Ensure continuity of care and follow-up planning
- Complete documentation and maintain accurate patient records.
- Process prescriptions and respond to clinical queries.
Poole North Primary Care Network is a network of four GP practices working together to deliver excellence in services for its 52,000 patients across seven sites. Although each practice is autonomous, we are a close-working network employing over 40 clinicians and staff to help patients access services in a timely manner.

Qualifications- You must be a registered and qualified ANP with a Digital Badge or be willing to work towards one.
