Interlink is looking for a Wellbeing Coordinator to support individuals to improve their mental and physical wellbeing by connecting them with community activities, services and support.

  • Post title: Wellbeing Coordinator
  • Hours: 37 hours per week
  • Salary: NJC Scale Point 21 (£33,143), rising to Scale Point 22 (£33,699) on successful completion of 6 month probationary period.
  • Location: Community-based and remote across Rhondda Cynon Taf (office base in Pontypridd)

Using a “what matters to me” and strengths-based approach, you will work with individuals to identify their needs, build on their strengths and help them access practical and emotional support. You will work closely with community groups, GP practices and health, social care and housing partners across RCT.

Key Responsibilities
  • Manage a caseload and co-produce personalised wellbeing support plans
  • Support individuals to access community activities, services and volunteering
  • Promote independence and self-management
  • Carry out assessments, reviews and safeguarding checks
  • Be active in the community, supporting outreach and promoting the service
  • Maintain accurate records and community information
About You
  • Experience of person-centred, strengths-based working
  • Confident working in community settings and partnership environments
  • Strong communication and relationship-building skills
  • Comfortable using digital systems, social media and Microsoft 365
  • Full access to a car for work purposes
