Operational Lead Heart Failure Nurse Services
An opportunity has arisen for a highly skilled heart failure nurse to join this dynamic team as an operational lead. The role will work closely with the operational lead for cardiac rehabilitation services and the Consultant Nurse.
The role will operationally lead the provision of core Heart Failure Nursing services across ABUHB, ensuring the provision of high quality effective, evidence based and efficient care through effective leadership and operational management.
Provide dedicated and resourced cardiology outpatient sessions for new and follow up patients, working as an independent practitioner, managing own caseload of patients, providing a high-quality service that involves; examination, assessment, referral to diagnostics, interpretation of investigations, diagnosis, development of management plans and evaluation of care. The role will be central to research activities within the directorate and provide leadership and guidance to the wider team.

Operational
- In liaison with Consultant Nurse, Cardiologists and General Physicians maintain responsibility and accountability for all elements of the heart failure services.
- Provide line management for Band 7 team members
- Ensure financial aspects are considered
- Develop, initiate and monitor Value based health care
Clinical
- Maintain a case load of patients
- Make professionally autonomous decisions on patients presenting with complex undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem solving and clinical decision making to establish diagnosis and management plan.
Leadership
- Provide leadership across heart failure and cardiac rehabilitation services at both local and national level
Research
- Ensure the department is proactive in research activities
- Contribute/lead on research studies
- Create an environment for others to be research active
Education
- Develop education opportunities and strategies for the service.

Duties and Responsibilities
Clinical
- In liaison with Consultant Nurse, Cardiologists and General Physicians maintain responsibility and accountability for all elements of the heart failure services.
- Liaise with the multi-disciplinary team and put systems in place to ensure patient safety throughout the phases of Care.
- Maintain an independent clinical caseload of diverse cardiac patients by providing nurse led cardiology outpatient clinics.
- Make professionally autonomous decisions on patients presenting with complex undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem solving and clinical decision making to establish diagnosis and management plan.
- Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings.
- Clinical skills- clinical patient assessment, risk factor identification and management, examination of heart sounds, examination of lung fields, chest pain assessment and referral to appropriate diagnostic tests. Venepuncture. Interpreting and addressing blood results.
- Where the post holder is an independent prescriber, to prescribe safe, effective, and appropriate medication as defined by current legislative framework
