Advance Practice Clinician (Virtual Primary Care)

As a high-performance team member, you will help drive the adoption of next-generation virtual care and provide individuals with personalized direct access to comprehensive care, including urgent and primary care, pharmacy, mental health, and more.
We are looking for an Advanced Practice Clinician (APC), a multi‑state licensed practitioner, to provide high‑quality primary‑care services via telehealth platforms. The role includes prevention, acute care, chronic disease management for ages 3+, sexual health for adults, and adolescent care per state guidelines. The APC will collaborate with physicians, care teams, and specialty providers and contribute to patient population health management and quality initiatives.
Duties- Conduct virtual patient encounters via video visits, phone visits, and secure messaging.
- Complete comprehensive histories, virtual assessments, diagnose and treat per standards of practice, care, and protocols.
- Order, interpret, and act on diagnostic tests (labs, imaging) in consultation or per protocol.
- Prescribe medications and adjust treatment plans per scope of practice and organizational protocols.
- Develop and implement individualized care plans, including lifestyle/behavioral health coaching, disease prevention, referrals, and self‑management support.
- Provide health promotion, wellness, and patient education (e.g., immunizations, screening guidelines, lifestyle modifications).
- Coordinate care with other providers, health coaches, specialists, and ancillary services.
- Ensure timely documentation of visits, treatment plans, referrals, and communications in the electronic health record (EHR).
- Collaborate with multidisciplinary care teams (physicians, nurses, pharmacists, health coaches, social workers) to ensure integrated care.
- Act as a clinical resource and mentor for other clinicians or telehealth support staff as applicable.
- Stay current with clinical guidelines, the best telehealth practices, digital health innovations, and relevant legislation/regulatory changes.
- Ensure all patient documentation complies with professional standards, regulatory requirements, and organizational quality and policy standards.
- Maintain credentials, licenses, certifications, and required continuing education.
- Adhere to organizational policies on telehealth, prescribing, scope of practice, referral management, and patient safety.
- Master’s or Doctoral degree in nursing (for NP) or Physician Assistant Studies (for PA) or equivalent advanced practice degree.
- National certification (e.g., AANP/ANCC for NP, NCCPA for PA) and appropriate credentialing and active DEA licensure.
- Current and unrestricted multi‑state license as a Nurse Practitioner (NP), Physician Assistant (PA‑C), in state of residence, as well as TX, FL, and OK. Willing to expand licensure to other states.
- Proven experience (preferably 2+ years) in primary care or outpatient care; telehealth or virtual care experience strongly preferred.
- Ability to conduct and complete (including documentation) a minimum of two visits per hour.
- Available to conduct visits during routine business hours and flexibility to extend hours to include weekend and evenings as needed.
- Previous experience in a virtual or remote primary care role.
- Experience with remote patient monitoring (RPM) technologies, digital health platforms, and asynchronous telehealth workflows.
- Fully remote/virtual environment; high‑speed internet and secure workspace required.
- Occasional in‑person meetings/training may be required depending on the organization.
- Must adhere to telehealth best practices, maintain patient privacy, and manage remote workflows effectively.
- May require participation in virtual on‑call rotations, triage or urgent virtual visits.
- The organization may provide training/mentorship for virtual‑care best practices, remote patient engagement, and digital workflows.
- The role may evolve as telehealth technology and virtual‑care models mature.
