Job Summary: The Wound Care Nurse collaborates with physicians and physical therapy to coordinate quality, cost‑effective care for patients and families at Casa Colina. Duties include direct patient care, education, consultation, counseling, and case management.

• Provide direct patient care and wound management.
• Develop and implement patient care plans from admission to discharge or continuum of care.
• Coordinate, monitor, and redirect wound care in partnership with physicians to maximize outcomes within appropriate length of stay.
• Educate patients, families, and interdisciplinary team on wound care and prevention.
• Document and maintain accurate patient records.

• Registered professional nurse with a current and valid RN license in California.
• Bachelor of Science degree in Nursing preferred.
• AHA CPR/AED certification required.
• Minimum 2 years clinical experience in wound care or specialty area, or wound care certification (CWS, CWON, CWCA, or WCC).

• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with interdisciplinary team.
• Commitment to patient‑centered care and continuous improvement.
