Job Summary

The Lead Nurse plays a key role in delivering high quality patient care and leading the nurses within the practice. The post holder provides clinical leadership and oversight of nurses, supports the development and implementation of protocols, leads quality improvement initiatives. They are responsible for managing, developing, and supporting the nurses to ensure services meet professional standards and achieve positive outcomes.

This role requires clinical expertise alongside effective leadership skills to coordinate daily nursing activities. The Nurse lead works closely with GPs and the wider team to integrate nursing services across the practice, ensuring patient safety, best practice, and high standards of care.

The role includes direct patient care, clinical duties, and administrative responsibilities, supporting a holistic and patient‑centred approach. The post holder is also responsible for ensuring compliance with infection prevention and control and cold chain requirements.

The Practice aims to provide a primary base to support continuity and strong patient‑clinician relationships. However, flexibility is essential, and the post holder may be required to work across branch sites on a temporary or permanent basis. Core hours are 8am–6:30pm Monday to Friday, with extended hours until 8pm on weekdays and 8am–4pm on Saturdays.

We are looking for applicants who are able to undertake sessions across our five sites within Plymouth. Our aim is to operate an efficient traditional GP model, providing efficient and effective direct patient care.

Job Responsibilities

Role purpose
The Nurse Team Leader is a pivotal figure in steering the nursing team towards excellence in patient care. This role encompasses oversight of clinical nursing practices, enhancing nursing protocols, and leading quality improvement initiatives across the practice.

Role duties

  • Clinical and Operational Leadership
    :
    Provide clinical and operational leadership, oversee recruitment, rota management, performance and professional development, maintain clinical governance, and lead development of nursing policies and protocols.
  • Patient Care
    :
    Deliver direct patient care, particularly in chronic disease management, urgent care, and preventative health; support patient flow and care coordination; lead health promotion initiatives.
  • Quality Improvement
    :
    Lead quality improvement projects, conduct and facilitate clinical audits, use feedback to refine practices, and adhere to strict infection control protocols.
  • Educational Leadership
    :
    Design and deliver training programmes, mentor nursing staff and healthcare assistants, fostering a culture of learning and continuous improvement.
  • Administrative Functions
    :
    Manage nursing resources, oversee documentation, and contribute to practice meetings.
Person Specification

Essential Criteria

  • Registered Nurse with active NMC registration
  • Degree in nursing
  • Chronic disease management experience
  • Cervical screening experience
  • Extensive nursing experience including advanced practice skills in primary care settings
  • Proficiency in managing a wide range of clinical scenarios including chronic disease management, emergency response and preventive care
  • Demonstrated leadership experience capable of managing and developing a nursing team
  • Experience in clinical governance and implementing quality improvement processes within a healthcare setting
  • Excellent communication and interpersonal skills; ability to manage multidisciplinary team meetings and patient consultations effectively; strong conflict resolution skills
  • Proven ability to manage resources efficiently including staffing budgets and medical supplies; skill in policy development, staff training and performance evaluation
