Livewell Southwest are currently looking for 2 Band 6 Physiotherapist within it Discharge to Assess Pathway.

Working alongside Physiotherapist, Social Workers, , Nurses, and Community Care Workers are would be required to undertake functional and statutory assessments on behalf of Livewell SW.

As a Band 6 Occupational Therapist you will play a critical role in the Discharge to Assess multidisciplinary team.

To provide a specialist, high quality therapy service within Discharge to Assess Team

To provide specialist Occupational Therapy assessment, diagnosis, treatment and education to individuals who may have a range of conditions and presentations.

To provide advice, signposting, guidance and information to health and social care professionals, patients, carers, relatives, or other non-professional contacts.

To be an integral member of the MDT providing education, training and dissemination of information to all healthcare professionals.

To be an autonomous practitioner, holding a caseload and working without direct supervision.

To continue own professional development in line with HCPC regulations.
