- Conduct psychosocial and bereavement assessments for home care and hospice patients and their families, serving as a vital resource.
- Identify and address patient and family needs, including emotional, social, and financial concerns.
- Develop individualized social work plans of care in collaboration with the family and interdisciplinary team.
- Provide support and resources to families facing end-of-life care, including bereavement support and referral information.
- Connect patients and families with appropriate community agencies and financial resources.
- Identify and address patient and family risk factors, ensuring their safety and well-being.
- Facilitate effective family and team communication.
- Provide in-service education to staff and volunteers.
- Maintain timely and accurate documentation that meets Medicare skilled criteria and medical necessity.
- Actively participate in team meetings, in-services, and quality assurance initiatives.
- Contribute to discharge planning to ensure smooth transitions for patients.
- Education:
A Master’s degree in Social Work (MSW) from a CSWE-accredited institution. - Experience:
Previous social work experience is highly valued. - Communication
Skills:
Excellent verbal and written communication skills are essential for effective collaboration and patient interaction. - Compliance:
Must successfully pass required criminal and driving record checks, as well as a Bureau of Elderly and Adult Services check. - Transportation:
Reliable transportation, a valid driver’s license, and proof of car insurance with $100,000/$300,000 coverage are required due to frequent local travel.
