Join as an Admissions Nurse and become a guide to families during one of life’s most difficult transitions. You’ll meet patients in their homes or senior communities, assess their needs, and begin services with clarity and care.

• Completing admissions in homes or facilities across the metro
• Answering tough questions with empathy and expertise
• Collaborating closely with our care coordination and clinical teams
• Documenting accurately and promptly

• RN, licensed in MN
• Hospice, oncology, or palliative care experience required
• Strong communicator and natural educator
• Clinical confidence and emotional maturity
• Valid driver’s license and reliable car

• Flexible work structure with autonomy
• Strong support from an experienced clinical team
