We are seeking a Medication Therapy Management (MTM) Pharmacist for a casual / flexible position. Role includes patient-centered medication reviews and clinical support on an as-needed basis. This role focuses on optimizing medication use, improving adherence, and supporting chronic disease management through structured MTM services.

Key Responsibilities- Conduct Comprehensive Medication Reviews and Targeted Medication Reviews
- Identify and resolve medication-related problems (duplication, interactions, adherence barriers, cost issues)
- Provide patient education on proper medication use and disease management
- Document interventions clearly and accurately in the EMR (Epic)
- Communicate clinically relevant recommendations to prescribing providers
- Support quality initiatives related to medication adherence and safety
- Maintain compliance with MTM and regulatory documentation requirements
- Active MN pharmacist license in good standing
- Strong clinical knowledge of chronic disease states (diabetes, hypertension, hyperlipidemia, etc.)
- Excellent communication and patient counseling skills
- Comfort with electronic documentation systems
- MTM experience
- Ambulatory care or clinical pharmacy background
- Experience working with Medicare or value-based care programs
- Flexible / casual hours based on need. Very flexible schedule
- No dispensing or inventory responsibilities
- Remote or hybrid work options
- Hourly or per-consult compensation (based on experience and volume)
- No benefits (casual status)
