The Diabetes Education Specialist provides patient-centered education and support to individuals with diabetes and prediabetes. This role focuses on empowering patients to manage their condition through lifestyle counseling, medication education, and self-monitoring skills, while collaborating closely with clinical care teams to improve outcomes.
This is a casual position, great for someone needing flexibility in their schedule and who wants to work with great physicians and care teams in a value based clinic practice.
Key Responsibilities- Provide individual and group diabetes self-management education
- Educate patients on blood glucose monitoring, insulin administration, and device use (CGMs, pumps)
- Support nutrition education, physical activity, and lifestyle behavior change
- Reinforce medication adherence and safe use
- Identify barriers to care and coordinate resources
- Document education sessions and care plans in the EHR (EPIC)
- Collaborate with physicians, nurses, pharmacists, and care coordinators
- Support quality improvement and population health initiatives
- Stay current with diabetes treatment guidelines and education standards
Required:
- Bachelor s degree in Nursing, Nutrition and/or Dietetics
- Current national certification as a Certified Diabetes Care and Education Specialist (CDCES), or ability to become certified within 24 months of hire
- Strong communication and counseling skills
- Comfortable using EHR and patient education tools
Preferred:
- Experience in primary care, endocrinology, or value-based care settings
- Clinic-based or hybrid role
- Flexible or standard scheduling, to be discussed with candidates
