Position Summary

We are seeking a Medication Therapy Management (MTM) Pharmacist for a casual / flexible position. Role includes patient-centered medication reviews and clinical support on an as-needed basis. This role focuses on optimizing medication use, improving adherence, and supporting chronic disease management through structured MTM services.

Key Responsibilities
  • Conduct Comprehensive Medication Reviews and Targeted Medication Reviews
  • Identify and resolve medication-related problems (duplication, interactions, adherence barriers, cost issues)
  • Provide patient education on proper medication use and disease management
  • Document interventions clearly and accurately in the EMR (Epic)
  • Communicate clinically relevant recommendations to prescribing providers
  • Support quality initiatives related to medication adherence and safety
  • Maintain compliance with MTM and regulatory documentation requirements
Qualifications

Required:

  • Active MN pharmacist license in good standing
  • Strong clinical knowledge of chronic disease states (diabetes, hypertension, hyperlipidemia, etc.)
  • Excellent communication and patient counseling skills
  • Comfort with electronic documentation systems

Preferred:

  • MTM experience
  • Ambulatory care or clinical pharmacy background
  • Experience working with Medicare or value-based care programs
Work Environment & Schedule
  • Flexible / Casual hours based on need. Very flexible schedule
  • No dispensing or inventory responsibilities
  • Remote or hybrid work options
Compensation
  • Hourly or per-consult compensation (based on experience and volume)
  • No benefits (casual status)
