Overview
Seeking a Social Work Care Manager to support older adults and families through hands-on care coordination and in-person visits/appointments.
Responsibilities- schedule appointments/transportation
- conduct assessments
- create/update care plans
- coordinate services (home care/hospice/nursing)
- communicate with families/providers
- follow up on referrals/orders
- assist with Mass Health & Social Security applications
- maintain documentation
- BSW/MSW + active MA social work license (LSW/LCSW/LICSW)
- 3–5+ years social work experience (geriatrics/care management preferred)
- Strong communication, organization, and resource knowledge
