- Participates in the coordination and delivery of patient care in keeping with the professional role of a licensed Nurse Practitioner. Provides clinical care, including patient visits, examination, diagnosis, and treatment.
- Provides education to the patient, family, staff, and community as may be required from time to time.
- Coordinates, conducts, and participates in evaluation of Nurse Practitioner services rendered.
- Implements evidence-based practices to improve patient outcomes.
- Is available for consultation with Agency staff.
- Will have a written Collaboration Agreement with supervising Physician in keeping with community and state standards for professionally and properly performing the role of a Nurse Practitioner.
- Participates in interdisciplinary team conferences to assist in patient care review and planning as needed.
- Participates in providing direct patient care and the evaluation of palliative care patients in both the home and clinic settings.
- Available to act as a consultant to Physicians and other Nurse Practitioners concerning a wide variety of patient care issues.
- Offers advice, and is available as consultant to nurses and others, on medical interventions to relieve pain and symptoms of disease, prescribing or recommending appropriate medication, dosages, treatments, or other appropriate interventions.
- Documents orders and progress notes in patient chart in a timely manner.
- Available as on call consultant to other team members regarding patients’ physical conditions.
- Provides education through teaching, in-services, new employee orientation, and other interactions to enhance the overall clinical competencies of the Agency’s clinical staff with an emphasis on palliative care.
- Provides clinical staff with the latest updates and information gathered from seminars, lectures, publications, medical journals, and other sources related to palliative care.
- Serves as liaison between Agency, physicians in the community, and the medical community in general
- Participates in hospital and community education programs from time to time.
- Participates in routine record reviews of patients.
- Provides patient information to the Medical Director and/or other Physicians for their use in determining continued patient eligibility for palliative care.
- Uses applicable computer technology to perform job functions as required.
- Maintains medical records in compliance with documentation requirements and coding procedures. Gathers statistical data and prepares reports as needed.
- Participates in quality assurance activities, process improvement and evaluation of patient care services rendered.
- Participates in health-related research projects as assigned.
- Committed to Agency resolution against fraud and abuse.
- Knowledgeable in hospice and palliative care philosophy, LCD, and state and federal rules and regulations.
- Arrives to work and meetings on time and ready to work.
- Respects confidentiality of patients/families and other Agency employees.
