Hospice of the East Bay (HEB) is seeking a community-driven Healthcare Liaison who is passionate about helping others understand the value and impact of compassionate, high-quality hospice care.
The position is Monday–Friday, 8:30 a.m.–5:00 p.m. (Weekends as needed)
Hospice of the East Bay (HEB) is a community-based nonprofit hospice and palliative care organization serving patients in their homes and at our six-bed inpatient hospice facility. For more than 40 years, our interdisciplinary team has provided compassionate, high-quality end-of-life care, with bereavement support offered for at least one year. Through our sponsored care program, services are provided regardless of ability to pay.

Reporting to the Director of Community Engagement, the Healthcare Liaison promotes awareness and understanding of hospice and palliative care services throughout the medical and broader community. This role builds strong relationships with physicians, nurses, social workers, case managers, and community partners to support timely and appropriate referrals.
Key Responsibilities- Educate healthcare professionals and community partners on hospice and palliative care services
- Build and maintain relationships with referral sources, including hospitals, clinics, and residential care facilities
- Coordinate and participate in educational presentations, community events, and networking meetings
- Serve as the primary liaison for Residential Care and Assisted Living Facilities, including regulatory coordination
- Track outreach activities and report on trends and referral activity
- Minimum 2 years of recent hospice and/or home care experience
- Strong clinical knowledge and referral assessment skills
- Excellent communication and presentation abilities
- Self-motivated, organized, and able to manage multiple priorities
- Knowledge of Medicare/Medi-Cal and hospice philosophy
- Valid California driver’s license and current auto insurance
