Overview

Reliant at Home is seeking a Full-Time Home Health RN Admissions Nurse in the Reliant at Home Plano coverage area. Must have HCHB experience and strong documentation skills.

• Primary functions are to perform Hospice admissions in the patient home setting.
• Develop and document the plan of care in correlation with the assessment.
• Provide skilled nursing care as ordered by physician.
• Timely submission of all required documentation.
• Communicate with supervisor, interdisciplinary team, patients/families, and physician in the coordination of care process.
• Demonstrate commitment, professional growth, and competency.
• Meet productivity standards.

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse, current State Driver's License.
• Two years experience as a Registered Nurse in a clinical care setting and 2-3 years Full-Time Home Health experience.
• Reliable transportation. Valid and current auto liability insurance. Current CPR.
