Team Leader Community Health Worker (Emergency Department)
Location:
Children's Health – Plano, TX
Shift: Full-time Monday through Friday 8:30 am – 5:00 pm. Expect 1-2 weekend days/month and one flex day.

The Community Health Worker Team Leader leads front-line staff, facilitates communication, oversees orientation, evaluates practice with focus on quality and performance improvement. 70% shifts, 30% admin/leadership. Develops team members and manages operational needs.
Responsibilities- People Leadership: hiring, onboarding, scheduling oversight, performance management, professional development.
- Staff rounding and coaching.
- Performance evaluations and corrective action.
- Positive inclusive work environment, model leadership, just culture.
- Staff accountability for patient care standards.
- Employee engagement feedback and action plans.
- Operational Leadership: scheduling and coverage, orientation schedules, PTO, attendance monitoring.
- Staff licensing and certification compliance.
- Regulatory and quality compliance.
- Participation in leadership meetings and committees.
- Patient, family education culturally responsive.
- Collaboration with interdisciplinary teams.
- Advocacy for patients and staff.
- Knowledge of health resources and care coordination.
- At least 3 years related experience.
- Leadership experience preferred.
- High school diploma required; bachelor’s degree preferred.
- Current Community Health Worker/Promotor license in Texas required; instructor license preferred.
- Medical plan premiums covered after 3 years.
