Various part-time daytime, evening, and weekend shifts are available throughout Nassau County.

Community Habilitation is a service provided to individuals with developmental disabilities in their own homes or community settings. The person, working together with their family and Care Manager, will create a support plan that best meets the needs of the individual and their family.

Primary duties and responsibilities include, but are not limited to:

• Supporting and assisting with activities of daily living, including personal hygiene tasks such as bathing/showering, tooth brushing, bathroom assistance, and menses care (if applicable). Also includes dressing, feeding, shopping, housekeeping, and laundry skills.
• Providing companionship, supervision, role modeling, and instruction while ensuring safety in both home and community environments.
• Documenting service implementation and individual progress in the Medisked system within 24 hours of service delivery.
• Supporting individuals to become as independent as possible at home and in their community.

Qualifications:

• High school diploma or equivalent preferred.
• Prior experience supporting individuals with intellectual and developmental disabilities preferred.
• Valid and qualified NYS driver’s license, compliant with agency’s insurance standards.
• Comfortable transporting individuals in your personal vehicle.
• Excellent communication and organizational skills, with the ability to understand written, oral, and email directives.
• Access to a web browser to complete required documentation.
