Opportunity Overview:
* Working in the town of Plainfield, CT
* 15
-20 patients per day
* Full-time position

* Compensation and Benefits:
Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated income range of $158,000 to $186,000 annually with no cap on productivity bonus income potential. This full-time role is benefit eligible.
Responsibilities:
* Drive high-quality, safe, and cost-effective patient care in post-acute care environment
* Inpatient rounding in skilled nursing facility, inpatient rehabilitation facility, and long-term care hospital
* Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
* Work closely with hospice, palliative care, and home care agencies for safe patient transitions
* Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
Qualifications:
* Board certified family medicine (FM), internal medicine (IM), hospital medicine (HM), or emergency medicine (EM)
* Willing to train experienced candidates as well
* Licensed in state
