Physical Therapist

We are seeking two Physical Therapists to join a Skilled Nursing Facility in Placerville, California. This role focuses on treating adult and geriatric patients in post-acute rehabilitation and long-term care settings.

This assignment offers a consistent schedule, guaranteed hours, and a structured clinical environment. You’ll work alongside a collaborative therapy team focused on safe mobility, functional improvement, and patient-centered care.

Evaluate patients and establish individualized plans of care

Provide skilled physical therapy interventions focused on mobility and function

Monitor patient progress and update treatment plans as appropriate

Collaborate with nursing and other therapy disciplines

Complete documentation accurately and in a timely manner

Active California Physical Therapist license

Skilled nursing or post-acute experience preferred
