the nurse navigator is the clinical team support role that facilitates care management. The nurse navigator provides care navigation through daily prioritization of patient services and follow up with referral sources, primary care providers, patients, clinicians, and clinical leaders through the management of incoming communication to the agency.

requirement description :

• 2 years community health experience in a medicare certified agency.
• demonstrated clinical competency and current knowledge of the principles and practices of home care.
• demonstrated familiarity with dph regulatory standards preferred.
• demonstrated coaching/team building skills required.
• excellent interpersonal skills and ability to handle conflict required.
• demonstrated flexibility both in thought process and in time availability.
• associate's degree required. Bsn preferred
• current driver license (and insurance) required
• in order to effectively provide home care, they must have a working knowledge of home care reimbursement and regulatory guidelines.
