Location:
Pittsfield, MA 01201. The role includes on-the-road visits and remote services within Central and Southern Berkshire County.
- Provide immediate support and connect clients to long‑term care providers.
- Work with clients via telehealth or in‑home visits.
- Collaborate with community partners such as police, fire, adult protective services, and aging councils to ensure timely assistance.
- Travel within the service area.
- Master’s degree in social work, counseling, psychology, or human services.
- Licensure within six months and at least two years of post‑licensure experience preferred.
- Full‑time or part‑time positions available.
