Salary Range: $24.00 - $28.00 per hour.

Schedule:

Part‑time (20–30 hours per week). This position is contingent upon grant funds; employment will depend on the successful acquisition and continuation of the behavioral health grant.

Recovery Program Assistant

Essential Duties and Responsibilities ,

Competencies:

  • Data and Quality Management:
    • Train in the federal Red Cap portal for NOMS data, track completion of data entry, and assist with entry.
    • Manage quarterly and six‑month reassessments required by the program grant.
    • Collect and monitor data related to patient engagement, outcomes, and quality.
  • Program Coordination:
    • Implement administrative and coordination functions that enhance case management, resource linkage, and patient engagement.
    • Coordinate workflows within the CHP Recovery Team (primary care, counseling, psychiatry, MAT, recovery, and crisis intervention) and with outside agencies to reduce barriers to care.
    • Liaise between the CHP Recovery Team and the ACO case management team around housing and critical needs support.
    • Collaborate with internal staff and community partners to improve patient access to essential services.
    • Foster teamwork, shared purpose, and coordinated patient care within the recovery program.
  • Patient Care Coordination:
    • Coordinate patient visits and scheduling for medication for addiction treatment (MAT).
    • Monitor controlled substance prescriptions and documentation requirements.
    • Ensure MAT treatment agreements and controlled substance agreements are completed and filed appropriately.
    • Track injectable medication orders from participating pharmacies.
    • Maintain compliance with all documentation and follow‑up protocols.
  • Case Management and Care Coordination:
    • Manage releases of information (ROIs) and external referrals for behavioral health and substance use disorder services.
    • Communicate with outside partners to support smooth transitions of care and timely information exchange.
    • Follow up on patient needs related to housing, insurance, or social determinants of health in coordination with ACO/CHW case management and clinic management.
  • Group Facilitation and Patient Engagement:
    • Organize and support group sessions on recovery, mental health, and wellness topics.
    • Conduct outreach, recruitment, and attendance follow‑up for group programs.
    • Track participation and outcomes of group interventions.
  • Experience (Preferred but not required):
    • At least 3 years’ experience in outpatient health care (primary care, behavioral health, or substance use treatment).
    • Demonstrated success in case management, care coordination, or healthcare project work.
    • Strong communication, organization, and time‑management skills.
    • Proficiency in EMR systems and office software.
    • Team‑oriented with proven collaboration skills.
    • Ability to engage patients with empathy, cultural humility, and trauma‑informed care principles.
    • Familiarity with harm‑reduction and recovery‑oriented approaches.
    • Bilingual (English/Spanish).
  • Education and Training:
    • Associate degree or higher, or equivalent work experience.
    • Medical administration, medical assistant, or addiction studies training.
