The RN Case Manager will serve as primary contact person and coordinator for adults with disabilities in a group home setting in Berkshire County.

• Be part of a team to help individuals with disabilities live their best lives through community engagement, enriching activities, and good health.
• Provide staff training and support on medical issues and techniques as needed.
• Perform health care assessments and develop written health care plans.
• Consult with program staff on specific medical interventions, techniques and documentation requirements.

Current Registered Nurse license with at least one year experience. Some experience with people with developmental challenges preferred.
