This is a great position for someone that wants work/life balance.
- Complete all required documentation on individuals assigned, including day hab and HCSIS requirements (if needed) and AFC requirements.
- Perform heath care assessments and develop written health care plans.
- Administer all medications and treatments as assigned in day hab programs.
- Conduct in-service training in health maintenance for AFC caregivers, AFC staff, and members served.
- Any other duties as assigned by the Director of Family Services and Advocacy Coordinator.
