Key Responsibilities:

• Provide comprehensive nursing care to residents with IDD in an ICF setting
• Monitor health status, administer medications, and implement treatment plans
• Collaborate with a multidisciplinary team to deliver person-centered care
• Track and document patient progress accurately and timely
• Support residents' daily living needs, promoting safety and well-being

Required Skills:

• Valid Pennsylvania LPN license or license eligible
• Proven experience in Long-Term Care (LTC) settings
• Strong clinical assessment and problem-solving abilities
• Excellent communication skills, both verbal and written
• Ability to work independently and as part of a team
• Familiarity with state and federal healthcare regulations

Nice to Have

Skills:

• Experience working with individuals with intellectual and developmental disabilities
• Knowledge of ICF regulations and protocols
• Bilingual abilities are a plus
• Certification in CPR and First Aid

Preferred

Education and Experience:

• Licensed Practical Nurse with a current Pennsylvania license
• Minimum of 1-year LTC experience, preferably in an ICF setting
• Prior experience working with individuals with special needs

Other Requirements:

• Reliable transportation for travel within the facility or community as needed
• Willingness to work flexible shifts including possible weekends
• Certification in CPR and First Aid preferred
• Must comply with all health and safety regulations and policies
