Population Health Medical Assistant

SHHC is seeking an experienced Medical Assistant to join our Population Health team. The MA will play a key role in supporting quality initiatives, patient outreach, care coordination, and clinical operations. Responsibilities include scheduling appointments, pre-visit planning, participating in huddles, managing patient care gaps, and collaborating with the care team to ensure high-quality services. This position reports to both Squirrel Hill and Brentwood office locations.

Key Responsibilities:

* Schedule follow-up appointments, immunizations, and in-house testing.

* Conduct pre-visit planning and identify gaps in care.

* Participate in daily huddles and share key patient data.

* Outreach patients for preventive screenings and track progress.

* Manage inbox tasks, relay lab results, and address patient inquiries.

* Collaborate with providers, RNs, and other SHHC teams to support care plans.

Qualifications:

* 3-5 years of experience as a Medical Assistant (certification preferred).

* Knowledge of medical terminology, ICD-10, and CPT coding.

* Proficiency in Electronic Health Records (EHR).

* CPR certification required.

* Bilingual skills preferred.
