* Job Summary:

** The primary function of the Scheduler is to process all aspects of scheduling nursing visits via Home Care Home Base software system.
** Functions:
*** Schedule and appropriately document patient schedules in a timely manner.
* Participate in coordinating care with management and patient interdisciplinary team.
* Provide effective communication to patients, staff members, other health care professionals and referral sources.
* Respond to agency and patient needs in a professional and creative manner.
* Demonstrate commitment, professional growth, and competency.
* Promote Agency philosophy and administrative policies to ensure quality of care.

* Skills & Experience:

**
* ** Minimum 1 year of home health experience.**
* ** Excellent computer skills required.**
* ** Excellent interpersonal and organizational skills.**
* ** Basic knowledge of scanner/fax/copier equipment.**
* ** Phone etiquette skills required.**
* ** Ability to take direction with a multitude of tasks.**
* ** Typing ability 40-50 WPM.**
* ** Knowledge of Medicare regulatory guidelines.**
* ** A reliable source of transportation**
* ** Valid/current auto liability insurance is required.
