GENERAL DESCRIPTION

The RN Case Manager:
Victim Services (RN) is a key member of the Trauma Recovery Medical Home (TRMH) care team at CHC/SEK. This program provides long‑term, trauma‑informed care for victims of intimate partner violence (IPV) and sexual assault (SA) across a seven‑county rural service area in Southeast Kansas (Service Area). The RN delivers individualized, victim‑centered services, coordinating with medical, mental health, advocacy, and community partners to address the physical, emotional, and social impacts of violence.

This position provides both in‑person services at CHC/SEK’s Pittsburg locations and at Safehouse Crisis Center’s emergency shelter, as well as telehealth services for victims throughout the Service Area. The RN serves as the central hub of care coordination—conducting intakes and assessments, building care plans, facilitating follow‑up, and ensuring wraparound support for victims and their children. The RN also co‑facilitates survivor‑focused life skills workshops and peer support groups, working closely with the mental health provider to promote resilience, connection, and healing.

In addition, the RN engages in cross‑sector collaboration, serving as a liaison to law enforcement, victim advocates, protective services, and multidisciplinary response teams such as Sexual Assault Response Teams (SART). This position requires advanced clinical skills, knowledge of community resources, and the ability to collaborate effectively across healthcare, justice, and social service systems.

ESSENTIAL DUTIES
  • Clinical Care:
    Provide trauma‑informed nursing care to victims of IPV and SA, both on‑site and via telehealth. Coordinate medical and dental care, referrals to specialists, and follow‑up services. Partner with CHC/SEK Community Health Workers to address social needs such as housing, food, and transportation.
  • Care Coordination & Program Support:
    Perform comprehensive assessments and develop individualized care plans for victims and secondary victims (e.g., children). Maintain regular follow‑up to ensure continuity of care and adjust plans as needed. Co‑facilitate life skills and peer support groups, addressing topics such as healthy relationships, reproductive health after trauma, parenting after violence, and rebuilding self‑worth.
  • Program Development:
    Contribute to the creation and refinement of TRMH workflows and protocols. Track program metrics, patient outcomes, and service utilization to guide quality improvement. Identify gaps in service delivery and develop strategies to enhance victim support.
  • Cross‑Sector

    Collaboration:

    Represent CHC/SEK in community forums, SART, and Coordinated Community Response Teams. Work with community partners to strengthen victim identification and engagement in the justice process. Promote improved system‑wide responses to IPV and SA.
  • Compliance & Reporting:
    Support data collection and grant reporting requirements for state, federal, and internal use. Ensure adherence to HIPAA, safety protocols, and organizational standards for victim confidentiality.

  • Active multistate Registered Nurse (RN) license or must obtain within 1 year of hire. License must be held in nurse’s resident state.
  • Minimum five (5) years nursing experience, preferably in emergency, women’s health, or case management; experience relevant to community health, correctional health, or law enforcement preferred.
  • Demonstrated program leadership and cross‑agency collaboration experience.
  • Basic Life Support (BLS) certification through American Heart Association or obtain within 90 days of hire.
