• A valid and current Florida RN License.
• At least 1 year of Medicare Home Health RN Nursing experience.
• Current CEUs for HIV, Domestic Violence, and Alzheimer's.
• Active Level II Fingerprint Screening
• Current CPR/BLS certification.
• Updated TB/PPD.

• Conducting comprehensive OASIS assessments and developing tailored plans of care in collaboration with PCPs.
• Administering medications and treatments, anticipating patient needs, and making recommendations to optimize their well‑being.
• Ensuring timely and accurate documentation, and collaborating with our in‑office case managers.
• Empowering patients and their families with essential health instructions, coaching, and counsel.
