Position Title:
Counselor – Substance Use Disorder - PRP

The Counselor – Substance Use Disorder - PRP provides daily services to adults recovering from mental illness and substance use disorders. The Counselor will work directly with individuals in developing service plans, providing relapse prevention, psychoeducation, and skills training. The Counselor works with individuals, 1:1 and in groups to assist them in choosing and obtaining personal goals and developing the skills necessary for independence and success in their environment of choice and to ensure that supports are in place for individuals to work towards recovery and maintain long term sobriety.

Schedule:

Full time. 40 hours weekly. This person reports to the Program Coordinator. FLSA:
Non-exempt

Required Education, Knowledge and Skill Abilities to Perform

Essential Job Functions:

An associate s or bachelor s degree or Peer/SUD certification is preferred. Ability to work as a team player is necessary. A good driving record is required and is defined as having a valid driver s license with no more than 2 points. The ability to respect individual choice in the recovery process. Also, this position requires good verbal and written communication skills and intermediate computer literacy.

Summary of Essential Functions, Responsibilities and Duties

  • Complete contact notes, Individual Service Plans, Monthly Progress Notes, annual documents, and other documents as needed and/or assigned.
  • Assist individuals on goals and interventions as stated on the Individual Service Plan.
  • Provide day program coverage (on-site services), facilitate groups, and develop group lesson plans.
  • Carry caseload of individuals
  • Ongoing communication with rehabilitation team, including Case Manager, Residential Rehabilitation Program, Health Home, and Supported Employment to ensure continuum of care.
  • Provide off site services in environments of choice such as in the individual s home or in the community.
  • Provide medication management.
  • Facilitate client appointments and complete follow-up paperwork.
  • Utilize Electronic Health Record to record services and documentation.
  • Utilize computer and systems for external and internal communication, time and attendance.
  • Act as a partner in recovery to provide services such as relapse prevention, drug and alcohol education, and recovery sustainment through group and individual settings.
  • Assist with locating and attending self-help groups (AA/NA, etc.).
  • Administer and monitor random toxicology screens based upon individual needs as stipulated by the individual s service plan.
  • Perform other duties as requested by supervisors.
