We are expanding our Maryland team and seeking a Wellness RN who thrives in a supportive, patient‑centered environment.

The Wellness RN plays a key role in promoting client health, safety, and well‑being through in‑home wellness visits. This role is ideal for a nurse who values autonomy, meaningful one‑on‑one care, and a predictable 4‑day‑per‑week schedule.

• Conduct in‑home wellness assessments and routine health evaluations
• Monitor vital signs, medication compliance, and overall client status
• Provide education to clients and families on health maintenance and safety
• Document all visits accurately and in compliance with agency standards
• Collaborate with the care team to support individualized care plans
• Identify changes in condition and communicate promptly with supervisors
• Promote independence, dignity, and quality of life for each client

• Active RN license in the state of Maryland
• Minimum 1 year of nursing experience (home health or community care preferred)
• Strong assessment and communication skills
• Ability to work independently and manage a flexible caseload
• Reliable transportation for home visits

• Consistent 4‑day‑per‑week schedule
• Work time 8:30am‑2:30pm
