Een gemeentelijke organisatie in Zuid-Holland zoekt een ervaren Wmo-consulent om inwoners te ondersteunen bij zelfredzaamheid en participatie. Je voert gesprekken om situaties in kaart te brengen en zorgt voor maatwerkoplossingen. Vereist is een afgeronde hbo-opleiding met minimaal 3 jaar ervaring binnen het sociaal domein. Het aanbod omvat een dienstverband van 24-36 uur, een salaris van €3500 - €4800 bruto en goede secundaire arbeidsvoorwaarden.
