Our Community Advantage Rehabilitation (CAR) is Durham Region’s largest community based rehabilitative therapy provider. Our Vision is to help adults and children maximize their functional abilities within their environment and enable them to participate more fully in life’s activities and rewards. We are looking for Registered Physiotherapists to provide services to our pediatric/young adult clients residing in group homes within the community.

Your impact
As a Lifemark physiotherapist, you’ll be a vital member of a multidisciplinary team, empowering individuals to overcome challenges, improve their mobility, and achieve their health goals. You ll be more than a care provider; you ll be an educator, motivator, and problem-solver, lifting people up, meeting people where they are in their health journeys, sharing knowledge, and delivering top-quality, patient-centered care.
In this role, you will:
- Provide ministry funded and private clients, community-based Physiotherapy services within the pediatric/young adults client’s place of residence and/or through virtual services
- Provide physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of gross motor skills, co-ordination, neurological, developmental, mobility aids, etc.
- Complete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and College of Physiotherapists of Ontario standards
- Develop personalized, evidence-informed care plans to support individual rehabilitation and wellness goals
- Collaborate with a multidisciplinary team to coordinate patient-centric care and achieve optimal outcomes
- Build the next generation through mentorship and leadership (as assigned)
- Stay current with best practices through continuous learning and professional development
- Utilize modern tools, including e-charting and digital communication, to streamline your workflow
- Partner with therapy assistants and kinesiologists to maximize clinical impact
- Educate patients and their families on recovery, prevention, and self-management strategies, linking to community agencies/resources when appropriate
