Overview
The Trust is recruiting for Community Nursing roles within the South Hambleton and North Ryedale team. Bases include Kirkbymoorside and Pickering. The Community Nursing teams deliver high-quality nursing care to patients in their own homes, working with hospital, social and primary care colleagues to support patients to stay at home and avoid hospital admission. The geographic base varies, with teams in the City of York, Selby District, Tadcaster or more rural bases in the Pickering area and Easingwold.
Each day you will work autonomously, delivering patient care to meet patient needs holistically. The team is large, supportive and friendly; you will have the backing of the team even when working independently. The service operates 7 days a week from 7:30am to 8pm, with flexibility around late shifts and weekends. Night shifts are currently provided by a dedicated overnight team, with potential rotation to nights if desired.
CPD is expected, with opportunities to learn and teach others to meet patient needs in their own homes.
We welcome applications from fully registered nurses with an active NMC PIN and from pre-registered nurses due to qualify within the next 3 months. We seek enthusiastic, caring and motivated nurses to provide the best possible care to our patients. Graduating nurses joining our Trust receive an exceptional 12 month preceptorship programme, supported by a dedicated preceptor to help them attain competencies in their chosen clinical area.
It is essential that you are a car driver and have access to a vehicle for work; mileage is reimbursed and parking at community sites is free for clinical staff. South Hambleton and North Ryedale is one of our most rural teams, so please consider the locality and travel from home before applying. Education & Training
- Essential – Registered Nurse; holds or is prepared to work towards an NMC recognised mentor qualification
Desirable
- Registered and up to date mentor
- Diploma or degree in healthcare
Knowledge & Skills
- Post Registration experience of working within Community or hospital services
- Experience of clinical supervision
Desirable
- Relevant skills such as palliative care experience and care of older people

- Able to plan, deliver and evaluate programmes of care and promotion of health and well being
