The Permanency and Youth Services Unit supports transition age youth who are experiencing foster care. The Unit strives to offer youth centered supports and services in order to ensure young people are prepared to live outside of the foster care system. The Transitional Housing Coordinator will support housing opportunities for young adults in Extended Foster Care. They will work with community partners, contracted providers, DCS Specialists and youth to determine which housing option best meets their needs.
They will provide technical assistance to Contracted Provider, as well as oversight and monitoring of the programs to ensure a positive experience for young adults.
- Accept and assist youth in completing Self Assessment Application with youth. Take youth on site visits of housing options. Coordinate with placement administration for youth who have to return to licensed placements. Complete needed documentation in Guardian including Service Authorizations.
- Track housing census weekly. Providing general information about housing, screening of applicants for acceptance into various housing programs as well as verifying eligibility for such opportunities.
- Facilitating referrals and serves as advocate on behalf of youth to ensure provider is assessing appropriate the in services, and supports are in place for youth to reach their goals. Coordinate between DCS and Contracted Providers.
- Provide monitoring, training, site visitation.
- Participate in staffing's with providers, assist in the resolution of conflict. Determine if youth needs alternative housing option, work with the DCS Specialist and Provider on attainment of new living arrangement.

- Represent and make appropriate decisions for DCS when working as a liaison in regard to interactions with DCS Staff, other agency personnel, and community partners and providers

Experience working with adolescents or young adults.
Pre-Employment RequirementsBachelor’s degree from an accredited college or university in education, social work, psychology, counseling or other related fields with 1 year of experience in working directly with young people in Housing programs or Arizona's foster care system. (Master’s degree from an accredited college or university in one of the above mentioned fields will substitute for one year of experience.) or an Associates degree in an accredited college or university in education, social work, psychology, counseling or other related fields with 3 year of experience in working directly with young people in Housing programs or Arizona's foster care system.

Requires the possession of and ability to retain a current, valid state-issued driver's license appropriate to the assignment. Employees who drive on state business are subject to driver's license record checks, must maintain acceptable driving records and must complete any driver training (see Arizona Administrative Code R). Employees may be required to use their own transportation as well as maintain valid motor vehicle insurance and current Arizona vehicle registration.
