Hiring Salary Range: $46,800 - $49,800 per year

About UMOM New Day Centers: Founded in 1964, UMOM is Arizona's largest provider of services for families experiencing homelessness. Our mission is to end family homelessness by restoring hope and rebuilding lives. UMOM serves over 15,000 unique individuals across all our services annually, including families receiving support at the Family Housing Hub (Maricopa County's family coordinated entry system).

Position

Description:

The Family Housing Hub Specialist assists families experiencing a housing crisis by problem solving, identifying resources, and providing support and/or assessments through the community coordinated entry system at the Family Housing Hub (FHH). The position is responsible for providing client navigation services and screenings for families experiencing homelessness to divert them to safe housing options and/or connect them to the community emergency shelter waitlist.

Essential Duties and Responsibilities:

  • Direct Client Care:
    Provides resources, support, and information to households and assists in creating short-term plans to solve immediate housing crises.
  • Engages in problem-solving techniques and housing stability planning with households, including preventing entry into the homeless services system when appropriate.
  • Completes assessments with households in a professional, trauma-informed, and accurate manner.
  • Collects all required information from households to ensure appropriate connections to resources and housing.
  • Serves as liaison between households and partner agencies to assure continuity of care.
  • In collaboration with partner agencies, assists clients in accessing necessary documentation for program eligibility.
  • Assists with triage at FHH front desk and phones, greets clients, and provides high level screening during initial contact.
  • Family Housing Hub Operations:
    Staffs satellite locations and off-site screening events; maintains up-to-date listings of community resources; communicates program details to external partners; assists with office administration and data inquiries.
  • Supports program Coordinator with customer service by fielding client concerns and external partner inquiries.
  • Documentation, Data Quality, and Outcomes:
    Ensures data is complete, accurate, and on-time per UMOM data quality standards; focuses on diversion from the homeless system, data quality, and program efficiency.
  • Other duties as assigned by Leadership and/or Executives.

Qualifications and

Competencies:

  • Experience and

    Education:
    • High School or GED required
    • Bachelor's degree preferred, ideally in a related field
    • Minimum 3 years of experience working with at-risk populations and/or case management
    • Demonstrated knowledge of Motivational Interviewing and Trauma-Informed Care preferred
    • Knowledge of nonprofit sector; experience with underserved populations is preferred
    • Knowledge of community resources and understanding of community health and social service systems is a plus
  • Computer

    Skills:
    • Basic computer literacy required (data entry, web navigation, typing, and ability to learn new software)
    • Proficiency with Windows, Microsoft Word, Excel, and Outlook
    • Experience with Efforts to Outcomes (ETO) or HMIS preferred
  • Compliance:
    • Valid Level One Fingerprint Clearance Card or ability to obtain one
    • Ability to clear the Central Registry Screening from DCS and APS
    • Ability to pass all required background checks and drug screening (pre-employment and random as required)
    • Valid AZ Driver's License and driving record within UMOM policy
    • 50/100 level of car insurance coverage
    • This is a Safety Sensitive position

Working Conditions:

This role is typically performed in an interior, controlled office setting. Duties may require extended periods of time at a computer and standard office equipment operation.

Work Schedule: Monday–Friday, including one early evening shift, with flexibility for occasional nights and weekends as needed.

Background Screens: This position requires obtaining and maintaining a valid Arizona FPCC. UMOM covers the cost if you do not currently possess an FPCC. All employees must pass a Central Registry Background Check (DCS and APS).
