Job Summary

Case Manager III at U.S.VETS will provide guidance and support to veterans to achieve their highest level of independence. The role involves collaborating with VA personnel, community partners, and internal staff to develop and implement individualized plans that address housing, employment, education, financial, substance use, legal, family, and medical needs.

Responsibilities
  • Assesses veterans referred to the program for problems, needs, or barriers in the areas of housing, employment, education/training, financial, sobriety support/addiction treatment, legal, family/social, and medical/psychiatric.
  • Coordinates care with VA and community partners for eligibility documentation and mental/physical health services.
  • Facilitates client intake, compiles files, and ensures all program eligibility documentation is complete.
  • Collaborates with clients to complete Individual Action Plans (IAP).
  • Provides guidance on IAPs, intervention strategies, progress notes, and discharge planning to Case Manager I & II staff during weekly meetings and as needed.
  • Conducts crisis intervention and supports peers with client issues.
  • Maintains confidential client files, ensuring all documents are current, complete, signed, and properly ordered.
  • Documents progress notes in Data/Assessment/Plan (DAP) format and reviews IAPs monthly.
  • Conducts home visits (project‑based or scattered sites) to provide case management services.
  • Assists with quality management activities, data collection, and report preparation.
  • Provides specialized case management services and life‑skills groups/classes for veteran subpopulations (female, disabled, Iraq/Afghanistan veterans, etc.).
  • Conducts random drug and alcohol screenings and maintains HIPAA confidentiality.
  • Assists program leadership with auditing client documentation and program reporting.
  • Supervises student interns and provides training to new and existing staff.
  • Responds to urgent client issues outside of business hours as needed.
  • Accurately enters data into the HMIS system and other required databases.
  • Documents client referrals, maintains time sheets, mileage logs, client expenses, and other required paperwork.
  • Performs other duties as required, including driving company vehicles for client transportation when a valid driver’s license is held.
Program‑Specific Duties
  • For Transitional Housing Clients: focuses on transitioning clients to permanent housing, manages daily contact logs, monitors rent compliance, and locates housing units for scattered‑site programs.
  • For Permanent Housing Clients: ensures clients obtain and maintain permanent housing, assists families with education enrollment, and maintains housing stability.
  • For Workforce Clients: assists with workforce assessment and enrollment, develops employment development plans, facilitates job placement and retention groups, and provides career support services.
  • For SSVF Clients: conducts rapid rehousing services, develops Housing Stability Plans, coordinates child education enrollment, and ensures VA compliance for financial assistance.
  • For Aftercare Case Management: makes home visits, coordinates meal planning, tenant responsibilities, transportation, and case conferencing with service providers.
  • For Incarcerated or Recently Released Veterans: assesses housing and employment plans, provides individualized support, conducts programs within correctional facilities, and maintains background clearance.
  • For Counseling Services: facilitates group/list counseling, maintains required licensure or certification, and ensures compliance with state behavioral sciences licensing.
Qualifications
  • Master’s degree in Social Work, Psychology, Counseling, or related field.
  • Minimum of 2 years experience in social services or related field (preferred).
  • Certification in Substance Abuse Counseling preferred; required for counseling roles.
  • For roles serving incarcerated veterans: must obtain and maintain prison clearance.
  • For roles providing nursing or health services: must hold LPN or RN credentials.
  • Experience with homeless and/or veteran populations preferred.
  • Experienced in facilitating support groups preferred.
  • Strong oral and written communication skills.
  • Professional appearance and demeanor.
  • Ability to work independently and within a team.
  • Computer proficiency in Microsoft Office and Internet.
  • Valid driver’s license preferred; must meet company insurance requirements and complete driver training if required.
