- Phoenix is a Magnet facility with ongoing research in the unit, some nurse-driven and some through the OB residency program. A Clinical Nurse Specialist for Women and Infant Services continually translates research into evidenced-based practice. Within the L&D unit there are specialized roles nurses can train for after 1-2 years of labor and delivery experience, including the newborn delivery nurse and OB triage nurse. We encourage nurses eligible to take their certification exams after 2 years in Labor and Delivery and provide education to support success.
Position SummaryThis position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. It is accountable for the quality of nursing services delivered by self or others under supervision and utilizes specialized knowledge, judgment and nursing skills to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
CoreFunctions
- Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and ensures completion within appropriate timeframe. Reviews information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and policy.
- Formulates a plan of care, including the discharge plan, using assessment data and input from patient, family and health team. Initiates a plan of care based on patient-specific needs and the medical plan of care. Sets realistic, measurable goals in collaboration with the patient/family and considers physical, cultural, psychosocial, spiritual, age-specific and educational needs. Plans care with the multidisciplinary team and revises the plan as patient status changes.
- Implements the plan of care through direct patient care, coordination, delegation and supervision of the health care team. Provides care based on physician orders and the nursing plan of care, complying with policies, standards and regulatory requirements. Delegates appropriately and provides nursing supervision in care provision. Communicates clearly to other caregivers to promote continuity of care.
- Evaluates the patient’s and family’s response to care and teaching, and revises the plan as needed. Monitors progress toward goals in collaboration with the health care team and evaluates the effectiveness of patient teaching.
- Documents assessment, planning, implementation and evaluation in the patient record. Ensures documentation is legible, timely and compliant with policy, reflecting data, interventions and patient response. Notes physician orders accurately and promptly.
- Provides care based on best available evidence and may participate in research within clinical practice. Participates in unit or facility shared leadership. Supports education, role development and orientation of facility personnel, patients, families and visitors. Promotes growth through mentoring and precepting when appropriate.
