The RN is experienced in planning, organizing, and directing home care services with an emphasis on community health education. The professional RN builds from resources to plan and direct services to meet the needs of individuals and their family members within their homes and communities.
Responsibilities- Completes an initial assessment of patient and family to determine home healthcare needs, including a complete physical assessment and history of current and previous illness(es).
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Develops a care plan involving the patient and the family to establish goals based on nursing diagnosis incorporating therapeutic, preventative, and rehabilitative nursing actions.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the Provider (MD, DO, DPM, NP, and/or PA).
- Provides direct patient care as defined in the State Nurse Practice Act.
- Provides health care instructions to the patient, family and caregivers as appropriate per assessment and plan of care.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload as a part of a multi-disciplinary team; regularly attends case conference meetings in office to ensure all patient issues are communicated to the Home Health team.
- Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNA's and MSW's.
- Associate's or Bachelor’s degree with one year of home health care experience preferred.
- AZ Nursing Licensure in good standing.
- Current CPR/ BLS certification.
- Level 1 Fingerprint clearance card.
- TB test results within the last year.
- AZ Driver's license and proof of insurance with reliable transportation.
- Excellent verbal and written communication skills.
- High level of organization and comfort with autonomy.
- Broad scope of nursing and health care knowledge with the ability to problem-solve.
- Demonstration of strong clinical reasoning.
