Position: Home Health RN, FT - Scottsdale/Central Phoenix

Assisteo Home Health is looking for a Full-Time RN in the Scottsdale/North Phoenix area.



Job Description

Summary:

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with an emphasis on community health education/experience. The home health registered nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes or home environment.

Essential Job Functions/Responsibilities
  • Provides direct patient care as defined in the Arizona Nurse Practice Act.
  • Completes an initial assessment of patient to determine home care needs. Completes OASIS based on data obtained. Provides a complete physical assessment and history of current and previous illness(es). Performs a medication reconciliation.
  • Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
  • Develops a care plan, which establishes goals based on clinical diagnosis and incorporates therapeutic, preventive, and rehabilitative care.
  • Communicates with physician regarding the patient’s needs and reports any changes in patient’s condition; obtains/receives physician’s orders as required to coordinate plan of care.
  • Understands and practices all State and Federal Laws within the scope of practice.
Position Qualifications
  • Registered Nurse with current license to practice professional nursing in Arizona or qualifying compact state.
  • Two years’ experience is preferred, at least one of which is in public health or home care nursing.
  • Must have a valid driver’s license and be able to work in the United States.
  • Must have working knowledge of Microsoft Office Suite and ability to learn to use Electronic Medical Records Systems.
  • Demonstrates excellent observation, problem solving, organizational, verbal, and written communication skills along with the ability to handle multiple tasks using sound judgement and available resources with minimal supervision.
  • Current CPR certification.
  • Current fingerprint clearance card.
  • Reliable vehicle with current auto insurance.
  • Good standing with the Department of Motor Vehicles.
  • Must be able to pass a background check for pre-employment and ongoing checks per Agency guidelines.
Working Environment/Physical Requirements

Prolonged or considerable walking, standing, driving. Be able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity, hearing, dexterity, and attention to detail to perform required nursing skills. Must have ability to perform CPR.
