Responsibilities- Direct the day to day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long term care facility. Ensure all nursing personnel comply with the facility A0written policies and procedures.
- Review and update department policies, procedure manuals, and job descriptions; make recommendations for revisions.
- Plan shifts' services, programs, and activities with assigned nursing staff and support personnel.
- Ensure the Nursing Service Procedures Manual is current and reflects day to day nursing procedures; ensure compliance by nursing service personnel.
- Provide written and oral reports/recommendations concerning shift activities; cooperate with other resident services to maintain the resident 0total regimen of care.
- Ensure nursing staff compliance with their job descriptions and participate in the facility's quality assurance program and government inspections as requested.
- Review and update the resident 0discharge plan; assist in planning the nursing services portion of the discharge plan as necessary.
- Interpret policies and procedures to personnel, residents, visitors, and government agencies; admit, transfer, and discharge residents as required.
- Complete accident/incident reports; manage resident charge slips and the Daily Census Report; perform administrative duties such as medical forms, reports, evaluations, and charting.
- Protect resident information and prevent unauthorized access; maintain compliance with privacy and information system policies.
- Adhere to charting and documentation standards; receive telephone orders, transcribe physician orders, and ensure accurate nursing notes and incident reporting.
- Prepare and administer medications as ordered; verify resident identity; maintain narcotic records and report discrepancies.
- Order medications, supplies, and equipment as needed; ensure adequate floor stock and manage safety procedures for medications.
- Review medication cards for accuracy and adherence to stop orders; dispose of drugs per procedures.
- Participate in staffing decisions, performance evaluations, and staff development; supervise and lead nursing personnel on the unit/shift.
- Develop work assignments and ensure adherence to nursing care standards; meet regularly with shift staff to address issues and improve services.
- Ensure department personnel and residents follow policies; maintain good interdepartmental relationships and a warm, calm unit environment.
- Handle complaints, grievances, and disciplinary actions fairly; report occupational exposures and potential fraud.
- Manage computer workstations securely; conduct nursing reports and ensure proper shift handoffs.
- Inform and prepare for new admissions; supervise room readiness and physician rounds; coordinate diagnostic and therapeutic services as ordered.
- Consult with physicians, review treatment plans and orders, and ensure treatments are properly administered by licensed staff or trained aides as appropriate.
- Implement nursing objectives and conduct periodic checks on care plans and treatment administration; ensure care plans align with resident goals and needs.
- Coordinate social and activity programs with nursing schedules; notify physicians and next of kin regarding changes in condition; carry out restorative and rehabilitative programs.
- Inspect treatment areas for cleanliness and safety; perform procedures such as catheterization, dressings, lab specimen collection, vital signs, and monitoring of seriously ill residents.
- Ensure patients who cannot call for help are checked regularly; assist in transportation for discharged residents; communicate with families regarding outcomes and post-mortem procedures as required.
- Monitor safety regulations, PPE usage, hand hygiene, fire drills, and disaster preparedness; participate in infection control programs and report hazardous conditions.
- Ensure safe operation of nursing service equipment; maintain MSDS documentation for hazardous chemicals; supervise equipment use by trained personnel only.
- Review care plans daily; inform Nurse Supervisor of changes; ensure CNAs follow the care plan and refer to it when delivering daily care.
- Assist in planning and revising MDS and RAPs; monitor care plans to ensure appropriate goals and problem resolution.
- Report suspected fraud related to billing and cost reports as required.
